The Meadows at Ashland

Azria Health in Ashland, Nebraska, is a warm and caring skilled nursing facility that includes specialized rehabilitation services, as well as two dementia care units for residents with Alzheimer’s or other memory-related symptoms.
Our excellent therapy team works closely with our short-term residents to accomplish their rehabilitation goals after their hospital stay, illness or injury, so they can return home.
